I was demonstrating Dragon (in an earlier form – about 2006) and dictated “This is a 36 year old woman with an knee injury who comes to be evaluated.” It nicely typed, “This is a 36 year old woman with a knee injury who comes to be violated.” Oops…
Shiny is pathognomonic, and explains it all.
Bet they still billed a Level 4.
“She is ailing and lives in a rest home” came out as “She is an alien and lives in a restaurant.”
Of course, most of the garbage in the above note is there to meet the bullet-counting required by the AMA’s cash cow CPT.

My favorite Dragon error was “retired custodian” translated as “retard custodian”
